First Choice’s preventive home health care helps patients receive better care by avoiding complications, thus helping our physicians. Preventive care means fewer trips to the hospital, and this in turn produces a savings on Medicare dollars. With patients not going in and out of the hospital, our physicians gain from this by maintaining a well-established and credible reputation.

Outcome and Assessment Information Set (OASIS) - A group of data elements that represents core items of a comprehensive assessment for an adult home care patient. It also forms the basis for measuring patient outcomes for purposes of outcome-based quality improvement (OBQI).

Medicare and Medicaid
OASIS is a key component of Medicare's partnership with the home care industry to foster and monitor improved home health care outcomes and is proposed to be an integral part of the revised Conditions of Participation for Medicare-certified home health agencies.
